Case Details:

Before and 3 months after a revision rhinoplasty. This patient had a reduction rhinoplasty by a prominent surgeon in the Philadelphia area 20 years earlier. Please notice the nasal bridge which is scooped out and the nasal tip which was left massively over projected. Also, please note the overly long and irregular nostrils. The computer imaging tracing in blue shows that the goal was to obtain a srtraight bridge and de-project the tip and make smaller nostrils. An attractive, natural profile was achieved in the “after” photo.On the front view, the “before “ photo shows a very irregular, distorted tip with bossae “knuckle” formation and pinched and very irregular nostrils. The tip and nostril deformities were taken care of with cartilage grafts from the patient’s ears.The tracing on the right is made from the preoperative computer imaging done during the consultation. Dr. Pontell creates this tracing which he uses as a guide during surgery to accomplish the patient’s goals. This allows Dr. Pontell to get as close to the desired result as possible. Please compare the desired profile in the tracing to the actual “after photo.”
