Revision Rhinoplasty

When a primary nose surgery has produced an unsatisfactory result, a revision rhinoplasty can be performed to correct the existing issues, such as a nose too large, too small, asymmetrical, collapsed nose structure, or other complications.

Revision Rhinoplasty: Delicate and Complex

Dr. Jonathan Pontell is often called upon to perform this delicate and complex surgical repair. Revision rhinoplasty corrects unsatisfactory results from a previous rhinoplasty. The revision procedure may be needed for several reasons and may not necessarily be the original surgeon’s fault. Rhinoplasty results partially depend on the patient’s unique anatomy and healing characteristics, and revision may also be needed to correct an injury that occurred after a primary rhinoplasty.

What Is Revision Rhinoplasty?

Rhinoplasty is a procedure performed to alter the size or shape of the nose. Due to the nose’s central location on the face, many people notice their facial harmony is affected by the appearance of their nose. Rhinoplasty is one of the most complex facial surgeries and one of the most requested revision procedures. Dr. Pontell in particular has more than 5,000 rhinoplasties under his belt, and he and Dr. Weng are committed to producing the best possible revision rhinoplasty results. Rhinoplasty is a highly requested facial cosmetic surgery because it can improve the balance of the facial features by creating a nose that is proportionate to the rest of the face with an attractive shape. Many patients unhappy with their initial rhinoplasty results can take advantage of revision rhinoplasty to improve many aspects of their nose, including shape, size, angle, and function.

Candidates for Revision Rhinoplasty

Ideal candidates have waited at least one year from the original rhinoplasty procedure to ensure the nose has healed sufficiently and that the outcome from the previous surgery is fully apparent. It is essential that the patient is in good health when undergoing surgery and that they have realistic expectations. Patients who smoke will stop at least one month before and after surgery.

Revision Rhinoplasty Procedure

The vast majority of rhinoplasty surgeries that Dr. Pontell and Dr. Weng perform are open. This means that a very small incision is made in the columella, the bridge of skin that separates the nostrils. This small incision is connected to internal incisions, separating the skin from the nasal cartilage and bones and providing our surgeons with excellent visibility of the nasal anatomy. The nasal tissues are then rearranged and repositioned accurately into a more flattering, better-functioning position. In cases where additional cartilage or tissue is needed, our doctor will take it from a donor site such as the nasal septum or the ear.

Recovery

Revision rhinoplasty recovery may take longer than the recovery from the initial nose surgery simply due to the increased level of complexity and correction. A splint will be put over the nose for one week to help keep it straight. Patients can expect bruising to resolve over the first week and swelling to decrease gradually over the first four to six weeks. It takes greater than one year for all the swelling to resolve from revision rhinoplasty, as opposed to about a year for an initial rhinoplasty. Most patients can return to work in one week and return to normal activities within two weeks. Oral medication will be prescribed to help with any discomfort.

Results

Most patients notice a significant improvement in the size and shape of the nose once the cast is removed. Once healing is complete, patients can expect a nose that is more complementary to the rest of their facial profile and creates more pleasing facial harmony.
